It is possible to add up to 3 additional BMR 360 Cards. Simply follow the process below. This form also serves for those who wish to merge several BMR 360 cards under a single one. Some conditions may apply.
- The main cardholder is responsible for obtaining additional cards in one of our stores.
- The main cardholder is then responsible for completing the form below.
- A BMR 360 customer service agent will contact the cardholder to confirm receipt of form and complete the request.
Our Customer Service is available from Monday through Friday, from 8 a.m. and 5 p.m. A waiting time should be expected before an agent contacts you by phone or e-mail, but rest assured that all requests are processed as quickly as possible.
*Additional cards are invalid for expense accounts and contractor-type BMR 360 cards.
